Thanks for your review. Attached are the v61 patches.

I've addressed the comments and rebased patches as needed due to the
latest changes on pgHead. The patch-0002 test file name has been
updated from "043_invalidate_inactive_slots.pl" to
"044_invalidate_inactive_slots.pl".
