Done.

It's not much, but it's a start.  Some things to do:

* handle default collation (probably comes with CF entry 2256?)
* preserve versions of initdb-created collations in pg_upgrade
* ditch collversion and ALTER ... REFRESH VERSION and start tracking
versions dependencies per-index (etc)
