prairiedog just reported the identical symptom:

I'm thinking "timing problem" more than "kernel bug".  (I'd have also
believed "ancient IPC::Run bug", except that I assume conchuela is
running a pretty modern IPC::Run.)

I can poke into it tomorrow.
