Subject: [PATCH] Adjust _bt_insertonpg() comments.

Remove an obsolete reference to the 'afteritem' argument, which was
removed by commit bc292937.  Add a comment that clarifies how
_bt_insertonpg() indirectly handles the insertion of high key items.
